Passport Information
Applying for a U.S. Passport
Passport issuances are done by appointment only. Please schedule your appointment here https://evisaforms.state.gov/acs/default.asp?postcode=LGS&appcode=1 This includes the replacement of a stolen or damaged passport, or a passport renewal. If you are applying for more than one family member, each member must have his or her own appointment. Please note that if the forms linked below are not completed upon arrival at the Consulate, you may be asked to reschedule your appointment. Extra passport pages do not require an appointment.
U.S. citizen minors under the age of 16
- Child must be present.
- Child's official, original birth certificate.
- Previous U.S. passport.
- Two identical passport photographs. Photographs must be 2” x 2” on a white background.
- Both parents must be present with identification. Evidence of child’s relationship to parents/guardian is required. Where applicable, provide evidence of legal guardianship.
- If one parent is residing outside of Nigeria, that parent must complete and notarize Form DS 3053 in the presence of an American Notary Public or American Consular Officer giving permission to the other parent to renew the child's passport.
- In case of death of one parent, the custodial parent should submit a Death Certificate.
- In case of divorce, both parents' signatures or DS-3053 must still be presented unless one parent has sole custody. A divorce decree with custody authority granted by an appropriate court is required as proof of sole custody.
- Photos of the progressive aging and physical changes of the child if the photo shown on the old passport does not reflect current physical appearance. If the passport was issued to an infant, please provide photos from every age of the child if possible.
- Complete Form DS-11.
U.S. Citizen over the age of 16 who does not currently possess a ten year passport issued within the past fifteen years
- Complete Form DS-11.
- Evidence of citizenship - previous U.S. passport and birth certificate.
- Photos of the progressive aging and physical changes from previous passport to present.
- Other photo ID, i.e. Passport or National Identification.
U.S. Citizen over the age of 16 who currently possesses a ten year passport issued within the past fifteen years
1. Complete form DS-82
2. Your most recently issued passport that was valid for ten years, and issued within the past fifteen years
3. A courier may bring your application form for you, if you provide them with a signed affidavit allowing them to submit your passport application. They will still need to schedule an appointment, as indicated above.
Fees
Fees may be paid in U.S. dollars or the equivalent in Nigerian naira. Applicants using Form DS-11 pay the passport issuance fee (one only), the surcharge and the execution fee for each passport. Applicants using Form DS-82 pay the passport issuance fee (one only) and the surcharge.
- U.S. passport issuance fee: Under age 16 (Form DS-11) US $40
- U.S. passport issuance fee, first-time issuance: Age 16 and older (Form DS-11) US $55
- U.S. passport issuance fee: Age 16 and older (Form DS-82) US $40
- U.S. passport surcharge: US $20
- Execution of passport application: US $25
Reporting and Replacing a Lost or Stolen Passport
The following must be submitted by the applicant in person:
- Police report
- Two identical passport photographs. Photographs must be 2” x 2” on a white background.
- Proof of US citizenship such as a U.S. birth certificate, Report of Birth Abroad, or Naturalization Certificate.
- Evidence to establish identity, such as a driver's license with photograph, ID card with photograph, Nigerian or other passport.
- Completed Form DSP-64 and Form DS-11.
- Total fee of US $100, or the equivalent in Nigerian naira.
Extra Passport Pages
If your passport's visa pages are becoming full, but your passport is still valid and in good condition, you may apply for additional pages. Only one set of extra pages (24 pages are in each set) can be added to your passport at a time, but if needed, and if your passport remains in good condition, you may have up to two sets of extra pages inserted into your passport. Once this limit is reached, you will need to apply for passport renewal. Please note that extra pages cannot be added to an emergency passport. Form DS-4085 should be submitted along with the passport. The process takes approximately 45 minutes and is free of charge.
No appointment is needed to add passport pages to a valid full validity passport. A courier may bring your application form for you, if you provide them with a signed affidavit allowing them to submit your application.
Damaged Passports
Passports that are damaged or in poor condition should be replaced regardless of remaining validity. For security reasons, damaged passports may be retained by this office.
Changes of Name
If you have changed your name, you may apply to have this change reflected in your passport. Please note that as of September 26, 2005, U.S. passports will no longer be amended to change items appearing on the data page; you will therefore apply for a new passport in your new name. For any change of name you must provide original documents.
Emergency Passports
Emergency passports may be issued for emergency travel when waiting for the full-validity passport to arrive from the U.S. would cause the applicant medical, personal, financial or other hardship. Please provide proof of imminent travel (i.e., existing airline tickets) and reason for emergency (i.e., police report or medical report). The fee for an emergency passport in US $100 or equivalent in Nigerian naira.
